American Health Care Association and National Center for Assisted Living Ranks Acacia #1 out of 141 Arizona Long-Term Health Centers
PHOENIX (August 5, 2025) – Acacia Health Center, the onsite short- and long-term rehabilitation center at Sagewood Senior Living in North Phoenix, was recently ranked first out of 141 long-term care communities in Arizona. It also earned the American Health Care Association and National Center for Assisted Living (AHCA/NCAL) Bronze Award. 
For the first time in the health center’s history, Acacia was ranked number 1 out of 141 long-term care facilities across the state of Arizona based on the Arizona Department of Health Annual Survey outcomes.
Acacia was notified of this ranking through the AHCA and NCAL Long-Term Care Trend Tracker, a web-based service comparing communities based on survey results and quality of care.
“Only one community in the state of Arizona can hold this distinction at a time and we are honored to be recognized for our outstanding quality of care,” said Misti Valentino, Acacia Health Center Administrator. “This is a testament to the hard work and commitment of the Sagewood team and the effort we put in every day to ensure residents and their families are receiving the best possible care.”
Acacia was also the 2025 recipient of the Bronze – Commitment to Quality Award by the AHCA and NCAL for its commitment to improving the lives of residents through quality care.
This award honors the healthcare providers across the nation who have demonstrated their dedication to enhancing the quality of care for the nation’s elders and individuals with disabilities. Acacia Health Center is only one of three Arizona communities to meet the standards of AHCA/NCAL Bronze.
“Being recognized for both of these distinctions true honor,” Valentino added. “They instill a sense of pride amongst our residents and staff alike, and it demonstrates our commitment to continually evaluate and improve our performance and strive to reach a higher level of excellence in healthcare.”
Acacia Health Center at Sagewood currently serves 75 residents and has a staff of 150 team members. They offer short-term rehabilitation, long-term care and comprehensive rehabilitation services.
