SCOTTSDALE, Ariz. [May 20, 2025] – Cogir Senior Living, a premier senior housing operator, is proud to announce its new partnership with Curana Health, the largest provider of senior living primary care services in the country. This collaboration brings concierge-style healthcare services to select Cogir campuses, offering seniors access to comprehensive and prevention-focused care within their own communities.
The collaboration officially began with the opening of an on-site healthcare clinic at Cogir’s Park Terrace Senior Living community (in Phoenix, Ariz.) and the launch of in-residence healthcare services at Cogir of Alexandria Fillmore (in Alexandria, Va.). Additional communities in Maryland and Virginia will begin offering in-residence care over the next few months, including:
- Cogir of Potomac in Potomac, Md.
- Cogir of Old Town, Alexandria in Alexandria, Va.
- Cogir of West End, Alexandria in Alexandria, Va.
Future plans for expansion include additional communities in Arizona.
“We are thrilled to partner with Curana Health, as this collaboration aligns seamlessly with our commitment to serving seniors and enhancing their quality of life through best-in-class programs,” said Holly McMurray, Senior Vice President of Care and Compliance at Cogir Senior Living. “By introducing concierge-style care, Curana Health enables us to further our mission of making a lasting, positive impact on every resident we serve—while also providing invaluable peace of mind to both residents and their families.”
Dedicated to delivering personalized care, Curana Health’s physician-led clinical teams offer seniors a wide array of primary, secondary, and urgent care services, including:
- Routine preventive care
- Chronic disease management
- Sick visits
- 24/7 access to care teams
- Medication review and reconciliation
- On-site phlebotomy and imaging coordination
- Palliative care and specialty care referrals
- Comprehensive care coordination
This value-based approach to care has been proven to positively impact senior health outcomes, with several of Curana Health’s operator partners experiencing a 39% reduction in hospital readmissions, a 30% decrease in falls, 39% fewer instances of polypharmacy, and/or a 95% resident satisfaction rate.
In addition to bringing Curana’s healthcare services to their residents, Cogir will also be participating in its Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO): a value-based model that provides operators with shared savings opportunities for their role in supporting care coordination, reducing unnecessary hospitalizations, and optimizing health outcomes.
“We are honored to work alongside Cogir Senior Living, a forward-thinking operator dedicated to supporting their residents’ health, happiness, and dignity,” said Amy Young, DNP, MSN, FNP-C, Vice President of Operations at Curana Health. “On-site healthcare services allow seniors to receive treatment without leaving the comfort of their homes and communities, and it’s a privilege to provide this service for Cogir’s residents.”
