Sagora Leader Joins Mobile Gracepoint Care Summit Answering Families’ Biggest Senior Care Fear: What Happens Between Visits?

The gaps between care settings were the focus of a regional conversation in Mobile this March that brought together leaders across the care continuum. Sagora Senior Living Business Development Director Jennifer Freeman joined that conversation as a featured panelist at the inaugural Gracepoint Care Summit, held March 12 at the Innovation Portal on St. Louis Street.

Freeman was among five panelists selected for the summit’s centerpiece discussion, “Navigating Transitions Across the Care Continuum,” alongside representatives from Care Patrol, Amedisys Home Health, Personic Health, and the Alzheimer’s Group. The panel explored where seniors most often fall through the cracks during care transitions, how providers can communicate earlier and more effectively, and what genuine cross-sector collaboration looks like in practice.

Hosted by Gracepoint Home Care and attended by leaders across Home Care, Home Health, Hospice, Primary Care, Assisted Living, Long-Term Care, and Memory Care, the summit was designed to address care not as a series of separate services—but as a single journey families experience together. The event also featured a presentation from Sensi.AI on how AI-powered monitoring can provide real-time visibility between care visits—a technology Gracepoint has already embedded across its Gulf Coast home care operations.
