This collaboration brings healthcare services on-site for residents across all Western Home Communities locations and care levels—including independent living, assisted living, memory care, and skilled nursing.
The partnership includes:
- In-residence primary care services, including preventive care, chronic disease management, sick/injury visits, psychiatric medication management, comprehensive care coordination, 24/7 access to care teams, and on-site phlebotomy and imaging coordination.
- The future opening of a healthcare clinic at Western Home Communities’ Jorgensen Plaza for Well-being in Cedar Falls, IA, which will provide care for residents, staff members, and their adult family members.
- Medical directorship services for Western Home Communities’ skilled nursing facilities, including dedicated support for clinical teams, consistent patient rounding, as well as risk stratification and antipsychotic medication stewardship.
- Participation in Curana Health’s Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO): a value-based care model that provides operators with opportunities for shared savings (through the support of care coordination efforts).
Residents can utilize Curana’s in-residence and in-clinic healthcare services as primary care or as an urgent care complement to their established primary care practice.
This partnership is a natural progression in Western Home Communities’ long-standing commitment to innovation in senior care—including Iowa’s first “small house” nursing cottages exclusively for dementia care and offering all-private nursing home rooms.
