Accountable Care Organizations (ACOs) are fast emerging as the central vehicle for hospitals to meet health care reform’s twin goals of quality improvement and cost reduction. In defining the transition path to such new payment models and care delivery structures, a central management challenge is forming high-performance partnerships with post-acute and long-term care providers to promote cost-effective utilization and prevent unnecessary hospitalizations. This presentation details the state of the accountable care movement, hospitals’ approach to relationship development and how post-acute and long-term care providers can position themselves to optimize cross-continuum partnerships.
